NaviCare does not charge offices or patients.
NaviCare Election
Providers give permission and information to NaviCare to contact the patient/family.


NaviCare Communication
NaviCare Navigator, an individual with HIPAA and CHADIS training, will attempt to contact the patient/family within one business day, explain the process, and collect additional information as needed.
Referral Selection
NaviCare Navigator will attempt to identify one or more appropriate matches for the referral and then provide them to the patient/family.


Referral Follow-up
NaviCare Navigator, with or without CHADIS assistance, will follow up with the patient/family to determine the success of the referral and repeat as needed and accepted by the patient.
